If you have filled out this form any time after July 1 of the relevant year, there is no need to resubmit. Unite for Literacy - Growing Readers We imagine a world where all families with young children are inspired each day to create a better life for their children. We believe that books can be a source for that inspiration and few things are more powerful than a caring adult sharing a book with a child. The right book at the right time can increase a child’s sense of identity, increase their reading competence, and most importantly, it can inspire their inner drive to take responsibility for their own learning. If your child is no longer attending, you are responsible for notifying our office and we will withdraw your contract effective at the end of the current month. We do not prorate for a partial month and we do not hold a spot without collecting fees. If you choose to leave your child enrolled without attending, you are required to pay the contracted rates. No credit, refund, or make-up time is offered for absences. Arizona Department of Economic Security offers assistance to families who qualify.
